DVD/CD Order Form - order a Sermon DVD (movie) or CD (audio)
DVD's $10.00 each (movie) CD's $5.00 each (audio only)
To obtain a DVD or CD, follow these three easy steps:
STEP 1. Fill out this form. It will be emailed to the office.
STEP 2. Make your payment in advance in one of these four ways:
(A) By check (in the memo, write "DVD Purchase" or "CD Purchase" and the Date of the Sermon).

(B) Pay with cash, check or credit card in the office.
(C) Pay by credit card over the phone. Call the office with the information, 727-531-7704.
(D) On the day of the sermon, pay with cash, check, or credit card at the sound booth.
STEP 3. When notified by phone or email, stop by the office or sound booth to pick up your DVD or CD.
