DVD/CD Order Form - order a Sunday Service DVD (video) or CD (audio)
DVD's $10.00 each (video) CD's $5.00 each (audio only)
To obtain a DVD or CD, follow these three easy steps:
STEP 1. Fill out this form. It will be emailed to the office.
STEP 2. Make your payment in advance in one of these four ways:
(A) By check (in the memo, write "DVD Purchase" or "CD Purchase" and the Date of the Sermon).

(B) Pay with cash, check or credit card in the office.
(C) Pay by credit card over the phone. Call the office with the information, 727-531-7704.
(D) On the day of the service, pay with cash or check, in the Social Hall at the Tech Team sales table.
STEP 3. When notified by phone or email, stop by the office during the week or on Sunday at the Tech Team sales table in the Social Hall to pick up your DVD or CD.
